  1. Read the Mission Statement  of the CAYM
  2. Download and sign the Membership Agreement
  3. Download and complete the Membership  Application Form
  4. Complete the online Form below and send the Membership Agreement and the Membership Application along with the Statement of Faith of your Church/Organization to:
